Services

Our In-home services are designed to ensure each beneficiary reaches his or her maximum health outcome. They provide needed interaction and activities whilst helping to relieve stress and worry.

Skilled Nursing

Home is often the best place for healing. Surrounded by familiar settings, family and friends, you can relax and allow the healing process to begin. In order to aid in your recovery, your doctor may recommend medical care provided in your home. Whether you are returning home from a hospital stay or a skilled rehabilitation center, or whether you require skilled nursing or rehabilitation therapies, Direct Care Home Health provides a continuum of services and support at any stage of illness or recovery. Every skilled nurse is fully licensed, bonded and insured.

Private Duty Nursing

Private Duty Nursing offers one-on-one medical care for individuals who need ongoing attention due to chronic illness, disability, or complex medical conditions. Our nurses are available for hourly or extended shifts, providing a high level of personalized care.

Whether your loved one requires round-the-clock assistance or just a few hours of support each day, our team delivers consistent, compassionate care that brings peace of mind to families and enhances overall well-being.

Physical Therapy

Physical therapists (PTs) provide services that help restore function, improve mobility, relieve pain, and prevent or limit permanent physical disabilities of patients with injuries or disease. PTs work closely with patients and clients to restore, maintain, and promote their overall fitness and wellness for healthier and more active lifestyles. Patients may include accident victims and individuals with disabling conditions such as low back pain, fractures, head injuries, arthritis, heart disease, and cerebral palsy.

Chronic Disease Management

Living with chronic illnesses like diabetes, heart disease, or COPD can be challenging. Our Chronic Disease Management services provide comprehensive support to help clients manage their conditions, reduce symptoms, and avoid unnecessary hospital visits.

We focus on education, medication adherence, symptom tracking, and lifestyle guidance, empowering individuals to take control of their health while being supported by a caring clinical team.

Medical Social Work

Our medical social workers assist clients and families with navigating the emotional, financial, and logistical challenges that often come with illness or aging. From connecting with community resources to advance care planning, we provide valuable support tailored to each situation.

We serve as advocates and guides, ensuring that each client has access to the services and support systems they need to thrive physically, emotionally, and socially at home.

Home Health Aide Assistance

Our trained Home Health Aides offer essential non-medical support such as bathing, dressing, meal preparation, and light housekeeping. These services help clients maintain a sense of independence while receiving the assistance they need.

Beyond the practical help, our aides provide companionship and emotional support, building trust and comfort with each visit. Their presence brings both relief to family caregivers and dignity to those receiving care.
