Requirements for Employment
- Proof of Identity (i.e. Driver’s License, Valid Photo ID, etc.)
- Certification (+skills), DC License RN/HHA/LPN/PT/OT/SP
- TB (PPD) Screening Within six (6) months of application/Chest X-Ray (Valid for 5 Years)
- Police Clearance |Within the last two (2) months as to the date of application.
- Social Security card
- CPR & First Aid
- Physical Clearance
- HEP B Vaccine Records / Declination Form
